Patch to the newest version of UT2004, delete ut2004/alienswarm/system/alienswarm.ini, play a round of plain deathmatch, and try again.

Did you extract the files to the ut2004 directory?

Is alienswarmstartup.ut2 in your ut2004/alienswarm/maps folder?

Also, use the "Edit" button. Please. It's to the left of the quote button.

We aren't here 24/7 to help you. WAIT.

Go into ../UT2004/AlienSwarm/System and delete AlienSwarm.ini

If the subdirectory /AlienSwarm/ does not exist in the UT2004 Directory, you've installed the mod wrong.

Your UT2004 Directory should look something like this;

that just means it's hiding the extension because it's a known file type.

Find and run AlienSwarm.exe in .../AlienSwarm/...

if that doesn't work, do as we've said, go into .../AlienSwarm/System/... and delete "AlienSwarm.ini" or "AlienSwarm". It should look like a notepad.

A good idea would to be to turn on file extensions;

Tools -> Folder Options -> View Tab -> Uncheck "Hide Extensions For Known Filetypes"

It should've downloaded right along with the rest of the game. Perhaps your download got screwed up somehow?

To clarify: I sent my downloaded zip. I don't think that's what fixed it though, there was an alienswarm.ini in ut2004/system that needed deleting, and I think that made everything happy again.
